RESUMO

BACKGROUND: The current COVID-19 pandemic mainly affects the respiratory system;however, with the increase in cases worldwide, there is evidence of compromise at the cardiovascular level, which can manifest as acute myocardial infarction, myocarditis, pericarditis, myopericarditis, heart failure, cardiogenic shock, vasculitis, deep vein thrombosis, pulmonary embolism, ischemic stroke, acute arterial insufficiency, arrhythmias, and sudden death. CLINICAL CASE: A 70-year-old male patient who simultaneously presented multisystemic thrombosis manifested by cerebral vascular event, pulmonary thromboembolism, acute myocardial infarction and acute arterial insufficiency in the context of SARSCoV-2 pneumonia. CONCLUSION(S): In patients with COVID-19 there is a high thrombogenic potential secondary to blood stasis, hypercoagulability and endothelial dysfunction, which worsens the prognosis and increases mortality, mainly in patients who require ICU stay, so an adequate thromboprophylactic or anticoagulant scheme and follow-up in the convalescent phase must be provided to detect sequelae associated with COVID-19.Copyright © 2022 Comunicaciones Cientificas Mexicanas S.A. de C.V.. RESUMO

An important criterion that must be known in the nanostore sector (bodega, warehouse, kiosk, grocery store) is the way that the competitive environment (EC) is related to adaptability. The study of adaptability and EC in this field is a new topic that requires research, due to the importance of the type of competitive strategy adopted by an organization, since new adaptability practices can be integrated as a result. There are some studies with frequently applied theories in supply chain management, logistics and adaptability of nanostores, focused on the challenges they currently face. Our research is based on the fit contingency theory, to identify the extent to which the variables of adaptability and EC are related to the performance of Honduran nanostores. The research considers 143 nanostores implementing adaptability, as an important operations program for CE and performance. This study presents new insights and offers guidance for nanostore owners, managers, and academics, especially that adaptability can positively intervene the relationship between EC and performance in nanostores.

BACKGROUND // Knowledge of social and gender determinants, which influence the places where people are exposed to COVID-19, may be relevant in the development of preventive and control strategies. The aim of this paper was to determine the context in which COVID-19 cases were infected (household, work/labor, health, social-health, and social-leisure settings) according to country of origin, occupational social class and gender, which is essential in order to designing public health strategies. METHODS // A cross-sectional study of an epidemiological registry of 56,628 COVID-19 incident cases was made, whose exposure/ contagion setting was studied according to the previous variables from June 15 to December 23, 2020, in the Region of Murcia (Spain). An exact Fisher test was used to study the distribution of COVID-19 cases based on the above variables. RESULTS //The cumulative incidence was higher in people from Africa (5,133.5 cases/100,000 inhabitants) and Latin America (11,351.1) than in non-immigrants (3,145.7). It was also higher in women (3,885.6) than in men (3,572.6). It is noteworthy, that 53.3% of the cases with employment were workers in industry or construction, artisans, agricultural workers, or elementary occupations. In contrast, during the second semester of 2020, 41.3% of the employed population in the Region of Murcia performed such jobs. The household was the main exposure setting (56.5% of cases with a known setting), followed by social-leisure (20.7%) and work/labor (18.2%). The labor settings were more important in immigrants from Africa (28.4%) and Latin America (35.7%) than in non-immigrants (12%), inversely to social-leisure settings. Labor context was more important in women (19.6%) than in men (16.5%) and in manual workers (44.1%) than in non-manual workers (26.6%). CONCLUSIONS // The context in which COVID-19 cases were infected is different according to social inequalities related to country of origin, gender and occupational social class.

RESUMO

Objective: NA Background: Acute disseminated encephalomyelitis (ADEM) is an inflammatory disease of the central nervous system thought to be caused by environmental factors to genetic susceptible individuals, where there is autoimmunity towards myelin components. Given the wide variety of symptoms, etiology and mimickers, ADEM is a diagnosis of exclusion, making difficult a prompt diagnosis. The pathogenesis associated between COVID-19 and ADEM is unknown, however, it could be secondary to immune-mediated mechanisms or molecular mimicry, creating a neuro-inflammatory response. Design/Methods: A previously healthy 22-year-old Puerto Rican male presented to the ED with altered mental status, incoherent speech, imbalance, and dizziness of 2 months progression. Neurological examination was remarkable for slow mentation, positive right Hoffman, left leg weakness and bilateral sustained clonus. Brain MRI showed innumerable foci of increased T2/FLAIR signal intensity throughout supra and infratentorial gray and white matter, none showing contrast enhancement. CSF with evidence of high protein levels, WBC of 7 (100% mononuclear) and normal glucose levels. Extensive workup for evaluation of infectious, demyelinating, inflammatory and vascular etiologies came back negative. Patient with recent history of gastrointestinal symptoms, reason why COVID-19 IgG/IgM Rapid Test was performed with positive IgG results 2 months prior to our evaluation. Patient was treated with intravenous steroids and intravenous immunoglobulin, with marked clinical improvement. Results: NA Conclusions: Here we present a case of atypical coronavirus disease 2019 (COVID-19) manifestation of a Puerto Rican male patient that 2 weeks after detecting SARS-COV-2 on his blood neurological symptoms started to develop. This is the first case of a young Puerto Rican patient without any other comorbidities where an association of COVID-19 infection and ADEM was found. Since ADEM if a diagnosis exclusion, it is vital to being able to distinguish this syndrome when it comes to a patient with history of COVID-19 infection and vaccine for prompt management.

Introduction: The mandatory confinement in response to Covid-19 forced the closure of colleges and universities, leading professors to adapt their teaching strategies to digital media, These changes in teaching processes showed an increase in the workday, greater stress, fatigue, and difficulties in separating work from other occupations, which may compromise the physical and mental health of teachers. Objective: To describe the occupational balance of occupational therapy teachers at a university in Cali-Colombia, during lockdown due to Covid-19, and to establish the relationship between sociodemographic variables and the presence of symptoms of anxiety and depression. Method: A cross-sectional study was conducted with 32 occupational therapy professors. The information was collected using Google forms containing a sociodemographic questionnaire, the OBQ-E, PHQ-9 questionnaires, and the GAD-7 scale. Absolute frequency tables and percentages were made for qualitative variables, and quantitative variables were presented as mean and standard deviation. Bivariate analysis was performed using t-student tests, ANOVA, and Pearson's correlations. Results: 75.0% of participants were women. In occupational balance the average was 36.5;meanwhile, 31.3% manifested symptoms of anxiety and 43.8% of depression. Conclusions: Most of the professors were women, gender that was related to the care of older adults.
